In 2002, in response to a number of public enquiries, it was announced that the CMO would chair an investigation into the current system of clinical negligence in the NHS and to evaluate ways in which it could be reformed.

As part of this investigation, Alastair Gray, with Paul Fenn and Neil Rickman, were commissioned to undertake a population survey, a literature review and a modelling exercise.

A consultation document – “Making Amends” – was published in 2003, and a final set of recommendations was published in 2007.
